Racial and Ethnic Approaches to Community Health (REACH 2010)

The AAHC is one of 44 sites nationwide to receive funding through the Centers for Disease Control and Prevention (CDC) Racial and Ethnic Approaches to Community Health (REACH) Program. The CDC-funded program, "A Healthy Community Starts with You: Community-Driven Strategies to Lower Risk Factors for Cardiovascular Disease," (CVD) addresses the reasons African Americans die from CVD at higher rates than white Americans.

REACH consists of interventions that offer personal support, community access, prevention, and youth advocacy. In addition, our community action plan builds capacity within the community by training community health workers to sustain improved health behavior norms among the Oregon African American population.
